1. Upon enrollment, the following fees are payable:
a. One-time $50 registration fee
b. Deposit (equivalent to 1 month's fees - used to offset the last month of enrollment at Canopy)
c. Fees (for first full month + any pro-rated fees for current month)

2. By submitting all personal data listed on this form, you consent to Canopy Education collating, using, disclosing, and/or processing your personal data only for the purposes of your child's registration with Canopy Student Care. Your personal data will be kept in accordance with the PDPA and GDPR guidelines (if applicable).
